Sample IEP Forms
K-12 released the new CT state IEP forms just 5 days after their release by the Connecticut Department of Education Bureau of Special Education. Here are some examples of the forms you would edit onscreen in RoundUp and IEP.web. Note that state forms for referrals, notices and special permissions are also available online, such as the ED634 below. Just click on the form or forms you wish to download, print them and show your staff how easy it would be to use our products.



IEP Form 1 automatically populates the student demographic data, reducing time for your staff and maintaining consistency throughout the IEP.

Goal Form 7 is populated with the generic Goal and Objective descriptions from the K-12 master library which are then to be edited by your staff to be specific to each student and his or her IEP. The master library descriptions can be edited and added to become customized for your district's requirements, saving the staff the time it takes to enter each IEP Gaol/Objective data manually.

IEP Form 11 automatically calculates the Special Ed hours and the Time With Non-disabled Peers hours and percentage from the information your staff enters in the appropriate rows of the form, reducing math errors and highlighting crucial compliance related data.

Progress Updates are accessed from a list of students and edited using drop down menus on a single page for each Goal that shows the respective Objectives for that Goal. This saves your staff precious time by reducing the steps to search and save for each IEP.
